Follow the link to the year in school you will be participating in. After reading the instructions, print the forms listed and complete them. Make sure to have everything filled out, signed, dated and organized before sending anything to NNU. 

Each of these forms contains information that will make our task of taking care of you easier and more efficient once you are on campus. Our Sports Medicine staff appreciates the time and effort it takes for this process, and we look forward to meeting you upon your arrival on campus.

ARE YOU BEING TREATED FOR ADHD / BEEN PRESCRIBED RITALIN?
 
If you have been diagnosed with ADHD or any other disorders associated with ADHD, and are taking prescription medications for these conditions, please review and print off the forms below and take to your prescribing physician.  The NCAA has a procedure to review and approve legitimate use of medications that contain NCAA banned substances through a Medical Exceptions Procedure. Completion of these forms will allow us to initiate this procedure if and when you are tested for stimulants.  Please feel free to contact us if you have any questions.

If you are in need of a physical, and will not be able to complete this before your arrival on campus, please let your coach know ASAP and we can help assist getting you to a physician.
